Our non-profit, population health-focused agency leads a Performing Provider System (PPS) across a 9 county rural region of upstate New York, under the NYS DOH DSRIP program. We contracted with Melissa to help us assess the current state of collaboration between clinical and non-clinical service providers in our region to address social determinants of health, and to develop a strategic communications plan to promote more robust and effective cross-sector collaboration as we move towards a value-based model for health care delivery and payment.
